2024 Annual Security Report Final

2024-2025 Adjustment to Housing

This form is used if a student experiences a housing plan change during the academic year which may result in a change in their cost of attendance (COA).

2024-2025 Citizenship Affidavit

This form is used when a student is not able to submit Citizenship Identity information in person to the Financial Aid Office. This document must be completed in front of a notary public, who affixes their unexipired seal to the form that is mailed in with the copies of the citizenship documents.

2024-2025 COA Increase Request

This form is use to request an increase to a student's cost of attendance (COA) for the academic year.

This form is used to verify your dependency status is correct on the FAFSA.

This form can be used if there are special circumstances and the student wants to appeal their FAFSA Dependency status.
